Getting to Know PB
PB lives with several health conditions that require continuous and comprehensive care. She needs assistance with airway clearance, has a gastrostomy for swallowing difficulties, a stoma for waste drainage, a suprapubic catheter for urine drainage, and requires full mobility support. These complex needs could impede her ability to partake in activities, but with the right care, PB has been able to enjoy a fulfilling life.
Home Visit and Care Integration
PB’s care plan is robust and meticulously designed to inform all aspects of her care. Regular meetings with multi-professional teams ensure that the best care plans are implemented and adjusted in real-time. This collaborative approach has allowed PB to enjoy a measure of good health, enabling her to pursue her interests such as attending music festivals and participating in fundraising activities.
Building Trust
The dedicated carers at Innovate Care Group play a crucial role in supporting PB to maintain a good quality of health. Their commitment and adherence to the care plan have fostered a trusting relationship, allowing PB to feel safe and supported. This trust has been vital in encouraging PB to engage in activities that she loves, promoting a sense of normalcy and joy in her life.
Challenges and Solutions
One of the significant challenges has been ensuring PB’s participation in various activities while managing her complex health needs. Innovate Care Group addressed this by conducting thorough risk assessments and creating detailed going-out checklists. These measures ensure that all necessary emergency equipment and medication are available, and the care plan is followed rigorously to maintain PB’s safety at all times.
PB’s Your Best Life (YBL) Journey
PB’s YBL journey includes participating in the Super Sensory challenge to fundraise for Soundabout, a charity that uses music to help Learning Disabled people with complex support needs thrive. Supported by her family, friends, carers, and the Soundabout team, PB engaged in sensory exercises over a 1km distance, experiencing sounds, tastes, and touches.
Looking Ahead
The future holds exciting plans for PB, including a dream trip to Jamaica, the land of reggae music she loves. Innovate Care Group is committed to supporting this journey by ensuring a robust risk assessment and coordinated care plan with all professionals involved in PB’s care. By continuing to provide high standards of evidence-based care through teamwork and partnership, Innovate Care Group aims to help PB achieve her dreams and live her best life.
