How we work

Our structured pathway is designed to deliver safe, person-centred care as smoothly and efficiently as possible. From the moment we receive a referral, we work collaboratively with clients, families and multi-disciplinary teams to plan and mobilise tailored care that meets individual needs. Throughout every stage, we maintain open communication and provide 24/7 out-of-hours support, ensuring we can respond quickly as needs evolve while maintaining the highest standards of care.

A new healthcare referral is received and reviewed by Innovate's experienced Clinical Team. A quote will be sent out for approval.

Our Clinical Lead Nurse completes a thorough, person-centered assessment and liaises with multi-disciplinary professionals where required. They will use this information to ensure that a seamless and holistic package of care is delivered.

We build a personalised care team through requirements matching and local recruitment. DBS checks and thorough compliance processes take place. We upskill Care Professionals through our mandatory and client-specific training pathway and arrange meet and greet introductions.

Tailored care plans and risk assessments are collaboratively completed and shared. Rotas and contingency plans are agreed upon and established.

All resources, equipment and devices are delivered to the home and the digital care management system is set to live. Shadow shifts and comprehensive competency assessments take place.

A Clinical Lead Nurse guides the delivery of care from the start. Monthly appointments are arranged for home visits, providing ongoing oversight and support.
