Mobilisation Pathway

Our pathway is designed to support smoother, faster mobilisation while maintaining high standards of care.

A new healthcare referral is received and reviewed by Innovate's experienced Clinical Team.

Our Clinical Lead Nurse completes a thorough, person-centered assessment and liaises with multi-disciplinary professionals where required. They use this information to ensure that a seamless and holistic package of care is delivered.

We build a personalised care team through requirements matching and local recruitment. We upskill Care Professionals through our training pathway and arrange meet and greet introductions.

Tailored care plans and risk assessments are collaboratively completed and shared. Care schedules and routines are agreed upon and established in place.

All resources, equipment and devices are delivered to the home and the task management system is set to live.

A Clinical Lead Nurse guides the delivery of care. Monthly appointments are arranged for home visits, providing ongoing oversight and support.
